The Alliance for Patient Medication Safety is collecting data pertaining to care of patients with diabetes. Since diabetes care does not always follow the literature-based published guidelines, there are often easily identified gaps in care. Participating pharmacists will identify the gaps and document them in order to improve the quality of care. Track and trend the gaps in your practice and contribute to a national study with the goal of improving protocols and minimizing common gaps.
