How do you become a Member?



 
  


Becoming a Member is easy. To get started, we need some basic contact information through an application.
You can utilize the link below to apply electronically, or Click Here to print one out. You may use a credit card for payment, in which case you can email or fax the form to us:

We currently offer 2 other options for becoming a member. First, if there are three or more of you working in the same pharmacy that wish to join, you can receive a 20% discount on your total dues. To take advantage of this offer, Click Here, complete the application for each interested party, and then send it in.
 
Second, we offer an entire pharmacy to join at the same time (up to 10 people) for $500.00. If there are more than 10 employees, each additional staff member is only$50.00.
