The Utah Pharmacists Association is accredited by the Accreditation Council of Pharmacy Education as a provider of continuing pharmacy education. All credit will be issued 60 days following the event. 

CPE Monitor 

CPE Monitor is a collaborative system used by ACPE, the National Association of Boards of Pharmacy (NABP), and ACPE-accredited providers for all pharmacists and technicians to record and track their CPE credits. If you have not obtained your NABP e-Profile ID number, go to service/ to set up your e-Profile. If you have any questions about this service or your number, please contact NABP customer support at 847-391-4406, Monday to Friday between 9:00 pm and 5:00 pm CT. 

Target Audience 

UPhA education sessions are designed for pharmacists and pharmacy technicians from all practice settings. 

Courses appropriate for pharmacist participation are designated by an ACPE universal activity number ending in the letter "P.” 

Courses appropriate for pharmacy technician participation are designated by an ACPE universal activity number ending in the letter "T.”
Review the ACPE information for each session to determine the amount of credit offered. 

Session Handouts 

To help preserve our natural resources, UPhA education sessions are paperless. However, to offer participants an enduring reference to the education content, the majority of the presentation materials are available at These presentations will remain online until December 31, 2017. 

Activity Type

All activity types at this meeting are Knowledge Based activities.

Partial Credit 

UPhA does not offer partial credit for any CPE activity. 

Learning Objectives 

Learning objectives for all CPE sessions are provided here, on the UPhA website,,, in the program event guide, and are provided at the beginning of each session. 


Financial support for individual education sessions is indicated on handouts and noted at the beginning of each education session. Faculty disclosures are provided upon request. 

CPE Requirements 

To earn CPE credit for a session accredited by UPhA, participants must actively participate in the entire session, submit an attendance code, complete an activity evaluation, and "claim” your credit using the online CPE submission site. Proof of your CPE credit will be available to you on CPE Monitor. UPhA no longer issues paper statements of credit. 

All continuing pharmacy education (CPE) participation will be recorded online for 2016. Recording credit online is quick and easy! 

• You may record your CPE credit online at after the meeting until December 31, 2076. 

• You will need your username and password. If you need this information you can request it at the meeting registration desk or using the forgot password option on

• You will need your NABP e-Profile ID to allow the CPE credit to be transferred to the CPE Monitor system. You will need an attendance code for each session that you attended. These codes will be provided to you in each session. 

• You can print your CPE transcript directly from CPE Monitor. Individual statements of credit from UPhA are not available. 

Grievance Policy 

Should any attendee of an approved CPE session be dissatisfied with the quality of the activity, a request in writing must be submitted to the UPhA Education Department at within 5 days of the conclusion of the session.
